Application for the Test Exemption

1. Those with the following diseases should apply for the test exemption:

Asthma, lung disease, hypertension, diabetes mellitus, kidney disease, arrhythmia, heart failure, arrhythmia, coronary atherosclerotic heart disease, cardiomyopathy, congenital cardiovascular disease, pericardial disease, etc. 

2. Those with the following injuries can apply for the test exemption according to the actual situation:

Inability to strenuous exercise during the test because of inflammation or injury of joints or muscles;

Inability to strenuous exercise after acute gastroenteritis surgery during the test;

Inability to strenuous exercise due to severe paronychia or after paronychia surgery during the test.

3. Other situation:

       According to students’ actual situation and the application materials submitted.

Notes for Application and Material Submission for Test Exemption

1. Process:

Assemble your application materials in one PDF naming as “Student Number + name + College”, and email it to corahuang@cuhk.edu.cn. You will receive an e-mail if approved.

2. Material:
   1. 1. Application Form for the Test Exemption (Please refer to Attachment 3)
      2. Medical certificate(s) by doctors from a Grade A tertiary hospital. (contains “Disease diagnosis certificate stamp”, both Doctor’s signature and signature stamp, none can be missing)
      3. Copies of medical history
      4. Copies of image data (if have) and the original for future reference
      5. Other relevant medical certificates

 (Those who have a history of the disease, please attach a copy of medical evidence of your past disease.)
